This is an exciting opportunity for a 12 month temporary post which could be a secondment opportunity. Care & Connect aims to promote the wellbeing of individuals and to prevent and delay the need for social care support. This is achieved by providing an alternative, proportionate response to need through harnessing the strengths and assets of people and their communities.
Care & Connect helps individuals connect with others, by developing their networks and connecting with others. We provide advice and information; signposting and assisted signposting to connect people to groups, activities and each other. We use an assessment tool to help identify needs and outcomes and help plan care and support.
The role of the community navigator is essential to the success of this service and will be working in the Hub based at Wallsend, as well as the community, networking and linking with other community resources, voluntary and professional. The community navigators will also be available to meet with people on a 1-1 basis and to develop their presence within a locality of the borough.
