We currently have a vacancy for a Health and Social Care Coordinator within Newcastle City Council’s Care Services teams, based at Welford Centre.
This post involves providing supervision and coordination within our care and support service. We are looking for applicants with a flexible approach who are committed to making a real difference to people’s lives.

Our staff work closely with health colleagues and must be able to demonstrate a strong commitment to promoting independence, health, wellbeing and choice to those who use our services. Maintaining the dignity of service users must be a priority at all times and successful applicants must be able to deliver care and support with great sensitivity.

We are looking to appoint a person who is enthusiastic, motivated and committed to working in a challenging yet rewarding environment. This post involves working with adults who have profound multiple learning disabilities with additional complex health needs, Autism and service users whose behaviour may challenge.

You will be expected to direct, coordinate and formally supervise colleagues, prioritise your own workload and delegate tasks to others. You will develop, review and evaluate care plans to assist services users in all aspects of daily living including personal care, assistance with meals and communication.

We will provide regular support, structured supervision and appraisal. In addition we offer training to meet national standards.

We are committed to protecting and promoting the welfare of children, young people and vulnerable adults and expect all staff to share this commitment. This post is working in regulated activity. If you are successful we will undertake additional recruitment checks which will include a check to see if you have had any criminal convictions, a check of police information and we will check the barred list(s). It is a criminal offence for a barred individual to apply for a job in regulated activity.
