The Adults & Communities Directorate, consists of four strands, Adult Social Care, Community Development, Neighbourhoods and Public Health. An exciting opportunity has arisen to join the Recovery and Independence Team on a temporary basis to support with Winter Pressures until the 31st March 2019. This will give individuals the opportunity to work with the team and gain experience which would be beneficial when applying for permanent posts
The Team provides a short term (for up to six weeks) reablement service to people in their own homes which enables people to regain as much independence as possible in skills for daily living ( personal care, meal prep, medication, mobility, use of equipment, accessing the community etc).
The team also provide a rapid response service to vulnerable Adults in crisis.
This is a Short Term Care at Home service (up to seven days) to support hospital discharge and prevent admission to hospital or a care home.
The rota is a two week pattern of four days a week and four time periods in the Day:
Week one: Monday, Tuesday, Thursday (11-14, 16-18, 19:30-22:00), Friday. Total 36hrs
Week Two: Wednesday, Thursday (am only 8-12) Saturday, Sunday. Total 32.5hr
Hours are worked in four separate time periods morning, lunch, tea and evening; 3hrs, 2hrs, 2hrs, 2.5hrs apart from Thursdays see above in brackets.
The successful candidate will have: numeracy and literacy level one or equivalent; have experience of working in a care/health setting; can drive and have the use of a vehicle.
