Grade 5 £22,658 - £24,964 plus sleep in payment and shift allowance
Full Time 37 hours

Together for Children is a Company in Sunderland which works on behalf of the Council to deliver children’s services in the city including social care, early help and education.

We are looking for candidates who are experienced in working with young people.

You will join a team of staff who deliver high quality care and support positive outcomes for young people by:

Co-ordinating resources and offering support to maintain or improve the wellbeing of clients.

Assessing and managing cases with a degree of complexity and risk taking action to co-ordinate resources and support to maintain or improve the wellbeing of clients.

Ensuring that the welfare of the children and young people is paramount.

Undertaking the role of link worker and developing residential placement plans, risk assessments and undertaking planned formal one to one work to ensure that individual care plans of children and young people are implemented and progressed.
