Senior Practitioner First Point of Contact Team
As the Senior Practitioner you will be supporting a team of social workers and community assessment officers within the First Point of Contact Team, providing sound advice, guidance and decision making around managing a variety of different initial contact enquiries into Adult Social Care. As part of the role you will be making decisions about appropriate responses to initial contacts into Adult Social Care, such as requests for assessments of need, enquiries regarding safeguarding issues, advice and information about local services, and responding to urgent or crisis situations.
You will have opportunities to provide supervision to both social workers and community assessment officers, offering support to staff, deputising for the team manager, and supporting in the development of the service. You will need to have an excellent understanding of the Care Act 2014, as well as other relevant legislation, to be able to analyse information, access and manage risk and have sound decision making skills.
We are looking for someone who would embrace the challenge of breaking down the barriers that prevent independent living. We are looking for someone who has passion and vision, who will harness multi-agency working, with a commitment to partnership working between Adult Health & Social Care and service users and their families.
This is an excellent opportunity, not only for individual development, but to be part of a very proactive and forward thinking team who will play a key part in terms of providing a first class response to those who make contact with Adult Social Care.
Senior Practitioner Ongoing Assessment and Intervention Team
As the Senior Practitioner you will be supporting a team of social workers in working with adults with a range of complex needs and who require ongoing social work intervention.
You will be supporting the Team Manager in driving the development effective practice of individuals as well as driving practice improvement across the service.
You will have a caseload of complex case work, working with individuals where Court of Protection work is required or there are complex safeguarding concerns involved in the case.
You will also deputise for the Team Manager when required, making decisions about the allocation of cases to social workers and community assessment officers and making decisions in relation to safeguarding concerns and other potential complex case matters.
Both posts have 31 days annual leave per annum and successful candidates will be able to make use of the council’s flexible working policy.
