The primary role is to support the delivery of a high-quality Reablement Service within the Intermediate Care Centre.
This service aims to prevent unnecessary hospital and care home admissions while supporting early discharge from hospital.
Key Duties
- You will work with individuals to help them regain confidence, independence, and daily living skills.
- You will support the delivery of a person-centred reablement service
- Assist individuals to regain or maintain independence in daily living activities
- Work collaboratively as part of a multidisciplinary team
- Monitor and support individuals with complex health needs
- Record and report baseline observations (e.g. vital signs)
- Assist with the supervised management of health conditions
- Contribute to safe discharge planning and ongoing community support
- Promote dignity, independence, and wellbeing at all times
Applicant Requriements
- You will preferrably be Educated to Level 3 standard (or equivalent)
- Have previous experience in a care setting
- Have experience with administering medication
- Have ability to work as part of a team and independently
- Demonstrate good communication and interpersonal skills
- Require a compassionate, patient-centred approach
This role operates on a shift pattern, including unsocial hours
Enhanced rates of pay are provided to reflect the working hours
