An exciting opportunity has arisen for a Team Manager within Short Term Support Services and front line services to facilitate working with a array of clinical experts focusing on client safety, discharge facilitation and admission avoidance.
The successful candidate will be leading an integrated service which incorporates Short Term Support Services, Northumbria Healthcare and Northumberland County Council. They will develop innovative ways of working, promoting change where necessary and helping achieve the organisations overall goals to deliver an excellent service, demonstrating a passion and commitment to deliver outstanding, high quality care to our service users.
The team is an integrated multidisciplinary team consisting of Physiotherapists, Occupational Therapists, Social Workers and Nurses as well as other professionals and care providers across acute and community services. Home care is a key component to the service which is being delivered.
The successful applicant will work closely with the Operational Managers and other Senior Leads, providing good leadership to optimise the performance and clinical excellence of the team. They will be responsible for ensuring that robust clinical and managerial supervisions are in place as well as delivering on performance objectives.
The service has an outstanding rating from CQC.
