The Older people’s community mental health team based at Woodside provides a social work service for Older People with Mental health issues, a diagnosed dementia and Adults diagnosed with early onset dementia in the Middlesbrough area, working with service users and their carers and working within discharge legislation.
The successful applicant will:
• Have sound assessment and care management skills
• Be able to work in a busy environment and to tight time scales
• Have excellent written and verbal communication skills
• Be committed to multi-disciplinary working
