The Hospital Social Work Team provides a social work service to Adults and Older People in the Middlesbrough and Redcar and Cleveland areas, working with service users and their carers during the discharge planning process.

We are currently seeking to recruit an experienced Social Worker with a minimum of 2 years post qualification experience to work as a ward based social worker in the Older People’s medicine wards at James Cook University Hospital (wards 11 and 12). Both wards have 24 beds and care for older people who are acutely unwell. Many of the patients on these wards will have a diagnosis of dementia along with complex medical problems and long term conditions. The wards pride themselves on delivering co-ordinated, person-centred care.

This is an exciting time to be part of the Trust’s friendly and forward thinking team in addition to being a part of an experienced and supportive social work team.

The role will involve:

Providing a social work service to residents of Middlesbrough and Redcar & Cleveland.
Liaising with allocated social workers and other local authorities to facilitate discharge planning.
Working as part of the ward team and participating in the daily handovers/board rounds.
Promoting the social work role and sharing knowledge of service provision and resources with other members of the ward team.
Being involved in exciting initiatives happening at present which include the roll out the Trust’s Frailty Strategy.

The successful candidate will:

Have sound assessment and care management skills
Be able to work in a busy environment and to tight time scales
Be committed to multi-disciplinary working
Be proactive and innovative in identifying patients requiring social work support on discharge
