Working as a Social Worker within our multi-disciplinary Integrated Community Delivery Team is both challenging and rewarding. Assigned to one of three place based locality teams you will undertake a range of strengths based interventions to ensure adults with care and support needs and their families and carers' achieve their desired outcomes. Working with people and their families and carers' remains a central part of what we do to ensure that they are able to make choices and decisions about their lives. Focusing upon individual outcomes; putting the person in control and making the best use of our resources are at the heart of our Adult Social Care Strategy. Our aim is to work with all our partners and deliver Practice to be Proud of to support the people of South Tyneside to Live Better Lives. Our Let’s Talk Together strengths based approach, based on 3 conversations allows us to build effective relationships to find out what really matters to the people we work with and enables us to get to know our surrounding communities, so we can link people with the right level of support.
