We’re looking for a skilled and committed Social Worker to join our Adult Learning Disability Team, working alongside social care officers, health professionals, and partner agencies to deliver safe, effective, and person-centred support.

Why This Role Matters
As a Social Worker in the Learning Disability Team, you’ll play a key role in supporting adults to live full, independent lives. You’ll work collaboratively with colleagues across Adult Social Care, the NHS, and community organisations, ensuring that care and support reflect the strengths and aspirations of the people we serve.
What You’ll Do
- Undertake Care Act assessments and develop person-centred support plans
- Manage complex cases and coordinate multi-agency interventions
- Support transitions, crisis planning, and safeguarding where necessary
- Provide professional advice on learning disability practice and legislation
- Promote rights-based, strengths-focused approaches
What We’re Looking For
- Social Work England registration
- Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding principles
- Experience in learning disability services and complex case management
- Excellent communication, analytical, and decision-making skills
- Commitment to person-centred, inclusive practice
