A Social Worker is required in the Durham and Chester le Street locality team to ensure that the social care needs of people are fully assessed and that services are delivered, monitored and reviewed in accordance with the agreed care plan.
Required qualifications: Professional qualification: Degree in SW, Dip SW, CSS or CQSW or equivalent plus HCPC registration.
The successful applicant for this post will be required to apply for a Disclosure & Barring Service Enhanced Disclosure.
