An opportunity has arisen for a qualified Social Worker to join the Community Health and Social Care Direct Team. This is a friendly and supportive team committed to person-centred practice which helps promote independence, wellbeing and choice.

The Community Health and Social Care Direct Team provides the first point of contact for Adult Social Care. Your role will include responding to crisis/emergency situations, completing adult assessments, carers assessments, and responding to safeguarding adult concerns, often in challenging situations. You will provide information, advice and support to prevent the need for long term support and promote independence. The role requires you to be flexible, adapt quickly and have the ability to make sound judgements, often in an emergency.

You should have a relevant social work qualification, such as Degree in Social Work, DipSW, CQSW, CSS or equivalent, and must have current HCPC registration. Ideally you will have post qualifying experience in an adult social care setting.

Professional support, supervision and appraisal will be provided by the Team Manager of the Community Health and Social Care Direct Team.
