An opportunity has arisen for a Social Worker to join the Adult Access Team, based at South Tees Health & Social Care Single Point of Access (SPA). The Access Team is the main point of contact for new requests for assessment, advice and information from the citizens of Middlesbrough.

The role is varied and will enable the successful candidate to develop a focus on screening, prevention, signposting, asset based approaches and reablement to support individuals to achieve their goals and desired outcomes. Undertaking assessments of individuals and their carers you will focus on meeting identified needs through a range of options including direct payments and commissioned services where unmet need remains.

The team has recently co-located to the SPA as a new development between Middlesbrough Council, Redcar and Cleveland Council and Health. You will be joining us at an exciting time as we develop services to maximise the benefits of integrated working between partner agencies.

Applications are invited from qualified Social Workers, registered with HPCP who are able to demonstrate:

• Post qualifying experience in Adult Social Care
• Excellent written and verbal communication skills
• Experience in managing a complex caseload
• Experience in positive risk management
• Enthusiasm and motivation to work with adults in need

Working as part of an established and supportive team with a wide range of experience we offer regular supervision and training opportunities to support and develop your practice.

Progression through the career link grade will be determined if and when specific criteria is fulfilled.
