The Hospital Social Work Team, based at James Cook University Hospital, provides a social work service to Adults and Older People in the Middlesbrough and Redcar and Cleveland areas, working with patients and their carers during the discharge planning process.
We are currently seeking to recruit a Social Worker to join our established and supportive team.
The successful applicant will:
• Have sound assessment and care management skills
• Be able to work in a busy environment and to tight time scales
• Have excellent written and verbal communication skills
Our team offers regular supervision and appraisal, excellent training opportunities and strong team identity.
Applications are invited from suitably qualified individuals (CSS, CQSW, DipSW, Degree in Social Work) applicants should be registered, or eligible for registration, with the Health and Care Professions Council.

Progression through the career link grade will be determined if and when specific criteria is fulfilled.
