The Hospital Social Work Team, based at James Cook University Hospital, provides a social work service to Adults and Older People in the Middlesbrough and Redcar and Cleveland areas, working with patients and their carers during the discharge planning process. We then provide short term care management support post discharge.

We are currently seeking to recruit a Social Worker to join our established and supportive team to work within the discharge pathway.

The post will involve working in care homes across Middleborough and Redcar and Cleveland, monitoring and reviewing short stay placements which have been arranged to support the discharge process. This will also involve participating in any future Discharge to Assess pilot schemes relating to care home placements.

This post holder will be expected to undertake a minimum of weekly visits to all service users placed in short term care with a view to supporting them to return to the community in a timely and coordinated way.

The successful applicant will:

• Have sound assessment and care management skills
• Be able to work in a busy environment and to tight time scales
• Have excellent written and verbal communication skills

Applications are invited from suitably qualified individuals (CSS, CQSW, DipSW, Degree in Social Work) applicants should be registered, or eligible for registration, with the Health and Care Professions Council.

Progression through the career link grade will be determined if and when specific criteria is fulfilled.
