i.e. pre-progression grade 9 scp 30 - 34
post -progression grade 11 scp 36 – 40
VACANCY NO: AHS/18/10A
Applications are invited for the above permanent post to be based within Intermediate Care + East Team.
The post holder will have a key role in ensuring an appropriate and timely response where assessments are required for clients experiencing short-term health crises; to minimise unnecessary hospital admissions and in supporting timely discharge from hospital.
The post holder will be required to contribute to the 8am – 8pm, 7 days per week, service delivery model, to include working occasional weekends and on-call arrangements over Bank Holidays
The teams enjoy excellent working relationships with colleagues in health settings and has a close working relationship with the other Integrated teams and statutory and non-statutory organizations.
Applicants will need to demonstrate:
- excellent time management and organisational skills;
- their experience and enthusiasm in working flexibly within a multi-disciplinary team;
- an ability to undertake holistic assessments of need, commission services, implement care plans, monitor and review.
It is anticipated that interviews will be held Monday 12th March 2018 .
Required qualifications:
Professional Qualification: Degree in SW, Dip SW, CSS or CQSW or equivalent. Plus HCPC registration or be applying for HCPC registration.
