Adult Social Care - Older Persons Community Mental Health Team

37 hours per week.

An experienced and highly motivated social worker is required to join the Older Persons Community Mental Health Team.

You will be required to undertake case work/ assessment and care management of people aged over 65 years who have been diagnosed as having dementia and/or other enduring Mental Health illness. Addressing the needs of carers is an integral part of this process.

The successful candidate will be a member of the Older Persons Mental Health Team based at the Campus for Ageing and Vitality and working in close liaison with health colleagues in the wider OPCMHT based at Centre for Health of the Elderly on the same site.

You should have a relevant qualification such as Degree in Social Work, DipSW, CQSW, CSS, or equivalent, and have current HCPC registration.

Experience working with adults with Dementia and enduring MH illness as well as experience of Safeguarding Adults are essential requirements of the post.

Professional support, supervision and appraisal is provided by the Team Manager of the OPCMHT.
