We’re looking for a skilled and committed Social Worker to join our Older People’s Mental Health Team, working alongside social care officers, and partner agencies to deliver safe, effective, and person-centred support.

Why This Role Matters

As a Social Worker in the Older People’s Mental Health team, you’ll play a key role in supporting adults to live full, independent lives. You’ll work collaboratively with colleagues across Adult Social Care, the NHS, and community organisations, ensuring that care and support reflect the strengths and aspirations of the people we serve.

What You’ll Do
  • Undertake Care Act assessments and develop person-centred support plans
  • Work within adult legislative frameworks.
  • Manage complex cases and coordinate multi-agency interventions
  • Support transitions, crisis planning, and safeguarding where necessary
  • Promote rights-based, strengths-focused, outcomes based approaches 

What We’re Looking For
  • Social Work England registration
  • Knowledge of the Care Act, Mental Capacity Act, and safeguarding principles
  • Excellent communication, analytical, and decision-making skills
  • Commitment to person-centred, inclusive practice
