Stockton-on-Tees Mental Health services for Older People has an opportunity for a temporary Social Worker to join the team, working with individuals who have a diagnosed mental illness and their carers.
The Community Mental Health Team is located at Lustrum Vale, 163 Durham Road, Stockton-on-Tees.
We are committed to promoting independence, choice and control and promoting Self Directed Support where people are eligible for social care services.
You will need to have an understanding of the needs of people with mental illness and their carers and have an enabling person centred approach, which promotes independence. The key responsibility of this role is to manage a complex case load.

Due to the requirement of frequent travel between venues across the Borough, a full driving licence and access to a motor vehicle is required for this role.
