Starting salary (SCP 25) £28,785

A number of exciting opportunities have arisen within our Community Locality Based Teams for qualified and highly motivated social workers who are enthusiastic and committed to working in a person centred /strength based way with customers. We also offer an attractive incremental progression.

Principal duties will be to complete assessments of need working to the National Eligibility Criteria as introduced by the Care Act and the arrangement of Care Support for vulnerable adults aged 18 and over, working closely with statutory partners, independent agencies and third sector partners.

Successful candidates will have:

Excellent communication and social care assessment skills, plus solid understanding of the relevant statutory frameworks and best practice.

The ability to work as an individual and within a busy multi-disciplinary team.

The ability to be flexible and self motivate.

The desire and motivation to work closely with health partners and other professionals.

Be HCPC registered.

Have a commitment to a flexible approach to their work to meet demands.
