Salary grade; Grade G, spinal point 34-36 £30,785 to £32,486 (dependent on experience and employees terms & conditions)

As part of the on-going work integrating services across health and social care in South Tees, we are seeking an experienced care or health professional to be either seconded or accept a short term contract until 31st March 2019 into this new role of Trusted Assessor for Care Homes in the Redcar and Cleveland area.
The primary responsibility of the Trusted Care Home Assessor is to act as an independent and unbiased assessor on behalf of Care Homes in relation to undertaking assessments to support and facilitate effective and timely discharges from Acute Hospital settings into Care Homes by working effectively with the Social Work and Discharge team whilst remaining independent.

The successful applicant will have either a background in Care Home management or substantial supervisory experience within adult social care or health, particularly in working with older people and be qualified to at least a level 4 QCF in health and/or social care or equivalent. Good working knowledge of CQC regulations and either current Social work registration with HCPC or current Nursing registration with NMC would be an advantage.
