The council is looking for a suitably experienced candidate to take on the newly created role of Continuing Care & Continuing Healthcare Joint Practice Lead. The main responsibilities within this role will be to support the council’s operational approach to the management of Children’s Continuing Care and Continuing Healthcare and joint funding agreements, to ensure compliance with national guidance. The post holder will be responsible for providing expert knowledge in the care management of children and adults, primarily with a learning disability, who are eligible for assistance under the Continuing Care and Continuing Healthcare frameworks, however you will also link in with and support other social care service areas, for example mental health and older people.
This individual will be supported and managed by the Service Manager, Operational Partnerships and Provider services, but will predominantly work closely within the All Age Disabilities service, providing specialist expertise to social care colleagues, social workers, team managers, service managers and other frontline professionals, regarding the CC and CHC process.

The successful candidate will possess a sound knowledge of the NHS Children’s Continuing Care and / or Adults’ Continuing Health Care guidance and have a good understanding of the MHA Section 117. We are looking for someone who is highly motivated, with high personal standards of self-discipline, able to demonstrate a flexible and organised approach to their work and be able to communicate effectively with a range of stakeholders and professionals.
