An exciting new opportunity has arisen for a full-time Social Worker based at the Rosedale Centre, Marske Lane, Stockton. The post reports to the Senior Practitioner in the Integrated Intermediate Care Service.

This new role has been created to support the Rosedale Centre to undertake its function as a short-term residential assessment and rehabilitation centre. The Rosedale Centre is a 44-bed facility owned and managed by Stockton-on-Tees Borough Council and registered with the national Care Quality Commission (CQC). The service enables safe discharge from hospital for those in need of additional physiotherapy, occupational therapy, and social care assessments. The service also supports admissions from the community when it is identified that a period of residential assessment or therapy is necessary.

The main duties of the role include:

- Oversight of all social care referrals made in respect of users of the service.
- Act as a link-worker for service users already involved with Social Work Teams to promote safe and timely discharge.
- Work alongside therapy and care staff to develop a comprehensive summary of needs.
- Liaise with relatives and other professionals to support a coordinated discharge from the service.
- Coordinate, facilitate and lead case reviews/discharge planning meetings.
- Work within legislative frameworks to deliver an effective service.

It is anticipated that an appointment will be made at Grade J in line with the level of experience, knowledge and skills required for this post.

Government legislation stipulates that all persons employed by a care home must be double vaccinated for COVID-19 (subject to medical exemption) from 11th November 2021
