The Wellbeing Facilitator role is at the hub of the Multi-Disciplinary Service focusing on both crisis intervention and early intervention and prevention services for the older population of the Borough. You will be working with people to identify a range of support to maintain their independence in their own home and reduce potential hospital admissions, where a care plan is not already in place.

You will need to be an experienced professional with a relevant Social Work qualification and have the ability, following training, to undertake a holistic health and social care assessment of support needs, including determining mental capacity and advocacy support requirements and a general clinical awareness, and working alongside colleagues from other professional disciplines.

This post has been identified as suitable for flexible working which enables the postholder to work 40% of their working time away from the office
