Your impact
This part time post will provide an integrated, child and young person focused, one team approach. Working creatively, to deliver family outreach support services to children, young people and families in need, across the Stockton-On-Tees locality. To plan, deliver and assess outcome focussed work with the aim of providing holistic, needs led support, in conjunction with the wider 0-19 service.

Your skills
You must have proven experience of ensuring children, young people and families receive
integrated, coordinated, multi-agency and solution focused support. You will need enthusiasm, a creative & flexible approach to your work alongside an ability to engage positively with a wide range of people and communities. A sound knowledge of child development and the assessment process is essential as is sharing our commitment to co-production. You will have experience of and the ability to act in the Lead Professional role and evidence the ability of working with children, young people and families, in an inclusive and accessible way.
