About the Role:
The Assistant Manager Community Systems and Coordination will work closely with the Support Coordination Manager and Support Coordinators to provide support with a range of tasks, becoming a valuable interface between operational social care practitioners, hospital discharge teams and service providers. You will assist the Support Coordination Manager to implement a systematic sourcing function which ensures timely access to appropriate services for people living in their community. You will support the team and work in collaboration with our partners in the delivery of services, prioritising urgent care and facilitating the flow of care packages through the system.
About You:
You will have experience of operational issues within a health and social care environment and be proficient in the use of Databases, have a sound understanding of contract management and quality assurance processes. With excellent communication skills you with be able to develop and maintain effective relationships with a wide range of stakeholders. You will need the confidence and ability to demonstrate sound professional judgements, when making decisions and have the ability to support real change.

About South Tyneside: South Tyneside has a rich heritage, and our history has played a major role in regional, national and even world history. Our Director of Public Health uses hypothetical children Jack and Emily to demonstrate the differences the two children born on the same day, but from different parts of the borough, and what impact that had on their health and wellbeing as they developed as children into adulthood. Jack was born in the ward of Simonside and Rekendyke and Emily in Cleadon and East Boldon Ward. Their journeys through life are quite shockingly different, culminating in jack expecting to live 75 years and Emily 87 years; a whole 12 years longer.
