Hartlepool Children’s Services is looking for a motivated and passionate Social Worker to join our Through Care team. This is a unique opportunity to be part of an intensive, specialist reunification team that supports children in care to return safely to their parents or family members.

About the Role
  • Hold a small, focused caseload of children identified for reunification.
  • Work closely with therapeutic practitioners and reunification support workers to deliver tailored, evidence-based plans.
  • Build strong, trusting relationships with children and families to understand needs and develop bespoke interventions.
  • Work closely with partner agencies such as Drug and Alcohol services, housing and adult mental health services to ensure support plans are in place.
  • Complete assessments, contribute to court work, and lead on applications to revoke care orders following successful reunifications.
  • Manage adolescent risk with an understanding of contextual safeguarding.

About You
  • Registered with Social Work England.
  • Strong knowledge of legislation relating to children in care.
  • Experienced in completing thorough assessments.
  • Strong knowledge of parenting interventions and support
  • Skilled in forming relationships, assessing risk, and delivering targeted interventions.
  • Confident working collaboratively within a multidisciplinary team.
