Our Adults and Integrated Care Service supports adults, carers and families to live healthy, independent and fulfilling lives. We are committed to helping people achieve the outcomes that matter most to them through early intervention, prevention and strengths-based practice.
 
We are looking for enthusiastic, compassionate and motivated Social Care Practitioners to join our busy and supportive Let's Talk Team, the front door to Adult Social Care in South Tyneside.
 
This is an exciting opportunity to join a service that is transforming the way support is delivered, placing prevention, wellbeing and community solutions at the heart of practice. We work alongside people as experts in their own lives, helping them identify their strengths, aspirations and support networks to achieve positive outcomes and maintain independence.
 
About the Team
The Let's Talk Team is often the first point of contact for adults, carers and professionals seeking support from Adult Social Care. Our focus is on having meaningful conversations that help people identify what matters most to them and explore practical solutions that build on their strengths, relationships and community connections.
 
We are committed to:

  • Supporting people to make informed choices and remain in control of their lives.

  • Delivering person-centred, strengths-based interventions that achieve meaningful outcomes. 

The Role
As a Social Care Practitioner, you will work directly with adults, carers and families across South Tyneside, delivering timely and outcome-focused interventions.
 
You will:
  • Undertake strengths-based conversations, assessments and support planning.
  • Work collaboratively with people to identify goals and achieve outcomes.
  • Promote independence, wellbeing and choice.
  • Complete intervention work to prevent, reduce or delay the need for ongoing support.
  • Support carers and recognise their valuable contribution.
  • Work alongside Social Workers, Occupational Therapists, health professionals and partner agencies.
  • Contribute to safeguarding activity, risk management and proportionate decision-making.
  • Utilise community resources, assistive technology, prevention services and local networks to support people creatively and effectively.
  • Maintain accurate records and contribute to high-quality practice. 

This is a varied and rewarding role where no two days are the same. You will need excellent communication skills, sound professional judgement and a genuine passion for helping people achieve the outcomes that matter most to them.
