2 Full-Time Posts - Central Middlesbrough PCN
This is a rewarding role supporting individuals to improve their health and wellbeing by connecting them with community services and other non-clinical support.
Working alongside Primary Care staff, voluntary, community and statutory organisations, you will help people address the wider determinants of health, such as loneliness, debt, housing and lifestyle issues, by providing personalised, non-medical support.
Applicants must have relevant experience of supporting adults with health and/or social needs.
