We’re looking for a dedicated and compassionate Social Care Officer to join our Adult Learning Disability Team, working alongside social workers, health professionals, and partner agencies to deliver safe, effective, and person-centred support.

Why This Role Matters
As a Social Care Officer, you’ll play a vital role in supporting adults with learning disabilities to achieve independence and wellbeing. You’ll work collaboratively with colleagues across Adult Social Care, the NHS, and community organisations, ensuring that care and support reflect the strengths and aspirations of the people we serve.
 
What You’ll Do
  • Carry out Care Act assessments and develop person-centred support plans
  • Support adults to live safely and independently in their community
  • Identify safeguarding concerns and take appropriate action
  • Provide advice, signposting, and short-term interventions
  • Work closely with social workers, health professionals, and voluntary sector partners 
What We’re Looking For
Experience in adult social care or learning disability support
Knowledge of the Care Act and safeguarding principles
Strong communication and problem-solving skills
Ability to work effectively within a multi-agency environment
Commitment to person-centred, strengths-based practice
