Are you a passionate and motivated Social Worker looking for a unique opportunity to make a real difference to the lives of young people, carers and their families?

North Tyneside Council is establishing an exciting new Transitions Team to support young people in their transition to Adult Social Care. The team will play a pivotal role in ensuring young people who may have care and support needs under the Care Act 2014, or who have acquired eligibility under Section 117 of the Mental Health Act 1983, experience a seamless, well-planned and person-centred transition into adulthood.

This is a rare opportunity to join a new service at the very beginning of its journey, helping to shape its culture, influence its development and create lasting improvements for young people, carers and families across North Tyneside.

About the Role

As a Social Worker within the Transitions Team, you will work with young people aged 14 to 25, supporting them to prepare for adulthood and achieve the outcomes that matter most to them. You will build meaningful relationships with young people, their families and carers, taking a strengths-based and person-centred approach to assessment, planning and intervention.

You will support young people with a wide range of needs and experiences, including those with learning disabilities, autism, mental health needs, physical disabilities, sensory impairments or a mixture of the above. Through effective assessment and planning, you will help young people understand their options, build independence, develop resilience and access the right support at the right time.

The role offers a diverse and rewarding caseload, providing opportunities to undertake assessments, develop outcome-focused support plans, contribute to transition planning from an early stage and ensure statutory responsibilities are met, whilst most importantly maintaining a focus on achieving the best possible outcomes for each young person.

Successful transitions rely upon effective partnership working. Within this role you will work collaboratively with Children’s Social Care, Adult Social Care, Education, Health, Commissioning and Housing to ensure a holistic approach. Your work will help young people, carers and their families navigate what can often be a complex period of change, by reducing uncertainty and ensuring they feel supported, informed and empowered throughout their transition journey.
