Adult Social Care is rapidly changing in South Tyneside. We are continuing our exciting journey of change which focuses on supporting our residents to live healthier lives longer, promoting independence and wellbeing, whilst delivering personalised services in partnership with them.

Working with people and their families and carers’ remains a central part of what we do to ensure that they are able to make choices and decisions about their lives. Focusing upon individual outcomes; putting the person in control and making the best use of our resources are at the heart of our Adult Social Care Strategy, “Promoting your Independence”.

One of the fundamental changes in our social care approach has seen us trial a “3 conversation approach” in a number of innovation sites, which allows us to have better conversations with people. This approach allows us to connect with people; respond at a time of crisis and plan for the longer term as necessary.
This is a challenging, yet rewarding position and you may be assigned to one of the above teams. You will undertake a range of interventions to ensure adults with care and support needs and their families and carers’ achieve their desired outcomes.
