Adult Social Care is rapidly changing in South Tyneside. We are continuing our exciting journey of change which focuses on supporting our residents to live healthier lives longer, promoting independence and wellbeing, whilst delivering personalised services in partnership with them.

Focusing upon individual outcomes; putting the person in control and making the best use of our resources are at the heart of our Adult Social Care Strategy.

To enable us to do this, we are changing our practice to have conversations to find out what really matters to the people we work with and we are getting to know what there is in our communities, so we can link people with the right support.
