Sunderland Care and Support is a Sunderland City Council owned trading company supporting a wide range of vulnerable people across the City of Sunderland.

Grace House provides short break accommodation to children and young people between 5 and 17 years of age who have a range of disabilities including learning and physical disabilities, Autism and behaviours which might challenge and require differing levels of support to meet a range of social and sometimes complex health needs.

Our short break service requires dynamic motivated Team Leaders with the ability to work in challenging environments. Team Leaders will need to be motivated to engage children and young people in line with positive behavioural support to enable a positive outcome and be willing to undertake training to support children and young people with a range of health interventions.

You will need to work effectively as part of a team, within a multi-agency framework, providing care and support to enable young people to achieve their maximum level of independence through appropriate physical, emotional and psychological support including: intimate personal care, support with a range of health needs, practical tasks, appropriate communication and community involvement.

As a Team Leader lead the team whilst on shift, providing care and support to enable children and young people to achieve their maximum level of independence through appropriate physical, emotional and psychological support including: intimate personal care, support with basic health needs, practical tasks, appropriate communication and community involvement.
