Further Information

An opportunity has arisen for a qualified Social Worker to join the Community Health and Social Care Direct Team. This is a friendly and supportive team committed to person-centred practice which helps promote independence, wellbeing and choice.

The Community Health and Social Care Direct Team provides the first point of contact for Adult Social Care. Your role will include responding to crisis/emergency situations, completing adult assessments, carers assessments and support planning, often in challenging situations. You will provide information, advice and support to prevent the need for long term support and promote independence. The role requires you to be flexible, adapt quickly and have the ability to make sound judgements, often in an emergency. The Community Health and Social Care Direct Team work in a multi-disciplinary environment with health colleagues and care workers. As such, the team has an ethos towards joined up working.

You should have a relevant social work qualification, such as Degree in Social Work, DipSW, CQSW, CSS or equivalent, and must have current HCPC registration. Ideally you will have post qualifying experience in an adult social care setting.

Professional support, supervision and appraisal will be provided by the Team Manager of the Community Health and Social Care Direct Team.

We are committed to protecting and promoting the welfare of children, young people and vulnerable adults and expect all staff to share this commitment. This post is working in regulated activity. If you are successful, we will undertake additional recruitment checks which will include a check to see if you have had any criminal convictions, a check of police information and we will check the relevant barred list(s). It is a criminal offence for a barred individual to apply for a job in regulated activity.
