The Adults & Communities Directorate consists of three strands, Adult Social Care, Community Development, and Public Health. Our aims are to continue to provide essential and highly valued services and to build capacity to enable local communities to make lasting change, improve quality of life and prosperity. As a directorate, we lead on developing a culture of co-operation and co-production, respecting and nurturing the skills and talents of local people and enabling them to fulfil their potential.

We are looking to recruit Social Prescribing Support Practitioners to support patients across two Primary Care Network areas in Redcar and Cleveland. There are six full-time permanent positions and an additional two, one year fixed term contracts.

This is an exciting new role and we are looking for enthusiastic, innovative, flexible and highly motivated people to help strengthen, develop, and work within our local general practices by delivering high-quality person centred support.

Applicants must have relevant personal or professional experience of working with vulnerable adults in social care, health, education, or voluntary and community sector. You must be able to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work.
