We are looking to recruit for a Reablement Assistant to participate in the provision of a Reablement service in the Intermediate Care Centre for a period of up to six weeks to help prevent unnecessary hospital and care home admissions and to facilitate Early Supported Discharge from hospital.
The role will be to participate in the provision of a Reablement service to enable individuals to regain or maintain their ability to live independently as far as possible.
