This is an exciting time for Middlesbrough as we develop ACT (Accessing Change Together) which integrates council staff and partner organisations and provides a range of support and interventions in relation to service users who need support with Housing Support and Homelessness, Domestic Abuse and Substance misuse.

ACT (Accessing Change Together) is an integrated service, which means that lots of support services are working together and using a shared case load management system. This will make it easier for service users to get access to services they need so they receive the right support and easier for council staff and partner organisations to work together and share information to improve outcomes for service users.

Working in collaboration with other services including Adult and Children’s Social Care and Public Health, to develop consistent pathways and effective processes. We have a permanent opportunity which has become available within ACT (Accessing Change Together) in the Housing Solutions Team for a post which will provide housing related support to rough sleepers.

This role will also support some of initiatives which form part of Middlesbrough Council rough sleeper action plan such as rough sleeper sweeps (rota basis) and drop in /outreach activity. This post will work alongside the Housing Solutions Officers to provide responsive needs led interventions with service users in order to help service users live independently and meet any practical social and emotional needs identified in assessments, or their personalised housing plan.

We are looking for someone who has experience of working effectively with a range of services and have the skills, passion, and resilience to work creatively with service users Whilst in post there will be development opportunities alongside flexible working with the option of a blended working environment (home and office where appropriate).
