We have been building stronger families since 1869 and today we work with more than 60,000 families in over 150 community based services, as well as supporting thousands more through national programmes and grants.
 
Within this role, you will provide an integrated one team approach to family outreach support services to children, young people and families in need across the Stockton-On-Tees locality. To plan, deliver and assess outcome focussed work with the aim of providing holistic needs led support in conjunction with the 0-19 public health service.
 
Your impact
You will provide high quality direct support to children, young people and their families; enabling them to access appropriate services, opportunities and receive the correct level of support. Delivering an integrated family outreach support service across the 0-19 age range, working in a multi-agency approach wherever possible.
 
Your skills  
You must have proven experience of ensuring families receive integrated, coordinated, multi-agency and solution focused support. You will need enthusiasm, a creative & flexible approach to your work alongside an ability to engage positively with a wide range of people. A sound knowledge of child development and the assessment process is essential as is sharing our commitment to co-production. You will have experience of and the ability to act in the Lead Professional role and evidence the ability of working with children and families.
