VACANCY NO: AHS/18/239A

We are looking to appoint a Care Facilitator to be responsible for negotiating and arranging appropriate packages of care for individual service users as specified by Social Workers/Care Coordinators. The post holder will be a member of the Care Facilitator and Direct Payments Team within Adults and Health Services.

The days of work will be Thursdays and Fridays.

This can be a high-pressure position and so the successful applicant will need to be good at prioritising and be able to work well as a team and individually when under pressure.

Required Qualifications: 5 GCSEs (A – C grades) or equivalent
