Social Worker -Care Co-ordinator
Temporary for 12 months subject to return of substantive postholder

Salary: £29,174 - £ 32,798- £ 34,373– £ 38,553 p.a.
i.e. pre-progression Grade 9/ post -progression Grade 11

Vacancy no: AHS/22/89A

Applicants are invited to apply for the above temporary full-time vacancy based within the 14-25 Navigation Team.

The post-holder will work specifically with 14-25 year olds with complex needs including learning and physical disabilities, autism, complex health conditions and mental health difficulties.

Applicants should be able to demonstrate an awareness of current issues in relation to Community Care for young people and adults with disabilities. Applicants should be able to demonstrate an awareness of the Care Act (2014), Mental Capacity Act (2005), Children Act (1989, 2004), SEND and multi-agency working.

Required Qualifications: Dip.S.W., C.S.S. or C.Q.S.W. or equivalent and hold Social Work England registration.

The successful applicant will be subject to a satisfactory Disclosure and Barring Service enhanced level disclosure check.
