VACANCY NO: AHS/22/222C

Applications are invited for the above vacancies based within the Hospital Discharge Teams.

The post holder will have a key role in ensuring an appropriate and timely response to support timely discharge from hospital.

The post holder will be required to contribute to the 8am – 8pm, 7 days per week, service delivery model, to include working occasional weekends and on-call arrangements over Bank Holidays.

The team enjoys excellent working relationships with colleagues in health settings and has a close working relationship with the other Integrated teams and statutory and non-statutory organizations.

Applicants will need to demonstrate:
- excellent time management and organisational skills;
- their experience and enthusiasm in working flexibly within a multi-disciplinary team;
- an ability to undertake holistic assessments of need, commission services, implement care plans, monitor and review.

It is anticipated that interviews will be held week commencing 12th September, 2022.
Interviews may be held remotely via Microsoft Teams.

Required qualifications:
Professional Qualification: Degree in SW, Dip SW, CSS or CQSW or equivalent. Plus Social Work England registration or be applying for registration.
