We are looking for a SWSA/Care Manager One for 3 months based at Dene Park House to work within the Adult Social Care Teams of the Community Services Business Unit. One of the primary functions will be to undertake initial assessments, statutory reviews of services users in receipt of a care and support packages. Additionally the role would offer support and frequent regular contact to services users, their families, or carers, who are not in receipt of support but are considered vulnerable.
This is a generic role to support service users across all disciplines to access community based services. The Care Manager One role is fast paced requiring a highly motivated individual to be able to work autonomously and be skilled in engaging with others to provide safe, high quality and timely service delivery. Teams provide a mutually supportive environments for staff to work within including regular supervision and monitoring.
