We have an exciting opportunity to join the Living Life service which is a unique partnership service between Middlesbrough Council and Middlesbrough and Stockton Mind. The post holder will be part of a team based at Middlesbrough and Stockton Mind main offices on Lothian Road in Middlesbrough.

The post holder will have excellent communication skills and be able to work in a fast paced, high demand environment delivering a quality assured service to adults struggling with emotional and mental health difficulties which are preventing them from engaging with community life and activities. As one of the main aims of the service is to support individuals to access mainstream activities and learning/volunteering opportunities in the community the post holder will be required to have knowledge of local services and activities available across Middlesbrough.

The post holder will be required to work in a person-centred way, assessing people’s needs, developing support plans and supporting people to set personal goals and achieve them.

Support will be provided in various locations to include the service users home, Council buildings, health care settings and community venues.

The post holder will receive a mandatory training package as part of this role.

Key duties of the post will include:

Working as part of a team of staff and volunteers, to provide a flexible and person-centred service which enables recovery, choice, independence, well-being and inclusion for people who experience mental health and emotional difficulties.

To support people to engage in mainstream community activities, including education, employment and voluntary work, through the delivery of individual action planning and community bridge building.
Engaging with service users in council buildings, health care settings, community venues and service users' own home as required.
