An exciting opportunity has arisen for the right person to become part of Middlesbrough Staying Put Agency within Social Care. The successful candidate will be responsible for supporting Middlesbrough residents to return home from hospital by assessing and referring for Telecare, Assistive Technology and other support services to enable them to remain independent in their own home.
Working closely with the A&E Therapies Team, Hospital staff and Social Work Teams within the hospital the candidate will provide information and advice on Connect, Telecare and other housing related support services which are available to residents, family members and professionals based within the hospital.
