Are you an enthusiastic, passionate, experienced professional who would like to be part of developing and shaping an exciting service for the Alzheimer’s Society?
This service will be based in specific GP surgeries in Gateshead and Newcastle providing support to people with a diagnosis of Dementia and their carers. You will work closely with Primary healthcare professionals as well as professionals across the health and social care sector in order to deliver a high quality service.
This role calls for a good understanding of dementia and proven ability to collaborate with external partners. You must also be an excellent communicator and listener who is able to work independently and inspire others. Excellent organisational skills are essential and you are likely to have experience of working with statutory and voluntary agencies. Add to that your ability to travel independently and work flexible hours when necessary, this could include the occasional evening and/or weekend and you will quickly establish yourself as a vital member of our team.
