An opportunity has arisen for a Social Worker to work as part of a multidisciplinary team, providing assertive outreach services to individuals with multiple complex needs, including mental ill-health, who are experiencing homelessness.

This is a friendly and supportive team headed by Changing Lives and funded by a Social Impact Bond. The project aims to address homelessness directly and to inform system change. The team is committed to person centred practice which helps promote independence, wellbeing and choice.

You will have a varied caseload that will include safeguarding work and engagement with service users and you will work closely with mental health colleagues within a multi-disciplinary framework to deliver good quality care and support. You will conduct assessments under the Care Act and link disengaged people in to mainstream services.

Applicants must have a relevant Social Work qualification such as a Degree in Social Work, DipSW, CQSW or equivalent. Experience of working in mental health and homelessness is desirable.

Professional support and supervision will be provided by the Team Manager of the Community Mental Health Team.
