The SEND 0-25 Service has brought together our Social Care 0-25 Disability Team and our Education SEN Team. It was created to ensure there was capacity to meet the needs of disabled children, young people, adults and their families in Hartlepool and the challenges this entails. It is envisaged that this will make transitions for disabled children, young people and adults more positive and effective and that the outcomes for them and their families will be improved.
We currently have a vacancy for a Social Worker within the team who will respond to and deal with referrals to the team and provide a comprehensive social work service to disabled children, young people, adults and their families on a medium to long term basis including assessment and support. We have developed strong links with targeted and preventative services working closely with our colleagues from partner agencies and intend to develop these further.

You should possess a social work qualification at a degree or diploma level (or equivalent).
