Applications are invited from qualified Social Workers for this 37 hour social work post.
The Transitions Team are enthusiastic, friendly and committed to improving outcomes for young people with disabilities aged 14 to 25 in Middlesbrough. This is an exciting opportunity to be part of providing comprehensive, multi-agency support to young people within this specialist team.
As a member of the team you will have a varied caseload and work with colleagues who are knowledgeable, supportive and passionate about their work. You will need excellent assessment, planning and care management skills, you will be focused and creative and enjoy meeting and working with young people and their families.
The successful candidate will work alongside colleagues in children and adult services to ensure forward planning of the transitional needs of young disabled people and will be involved in the development of services for this vulnerable group. The role will also involve liaising with professionals in Education, Health and the Voluntary Sector to ensure a holistic approach is followed to meet the needs of young disabled people.

The successful candidate will have:

• Relevant experience.
• Professional qualification CQSW/DipSW/SCC/Degree in Social Work.
• The ability to work as part of a team to ensure the effective delivery of service.
• A sound knowledge of Child Care and Adult Social Care Legislation, Social Services and Inter Agency Policies and Procedures and current development in Children and Adult Services.

Candidates must be registered with the HCPC and hold the relevant social work qualification.
